What is Amoxiclav?

Amoxiclav belongs to the group of penicillin antibiotics with the addition of clavulanic acid, which prevents the destruction of amoxicillin by beta-lactamase enzymes produced by some bacteria. This combination increases its effectiveness against bacteria resistant to other penicillins.

Amoxiclav is used to treat a variety of bacterial infections, including respiratory, urinary tract, skin, and soft tissue infections, as well as otitis media. In the form of “Amoxiclav 625mg (60ml)”, it is often prescribed for adults and children over 12 years of age to fight more serious infections or those that are resistant to other antibiotics.

Amoxiclav 625mg dosage for adults

Amoxiclav dosage depends on the patient's age, weight, kidney function, and the severity and type of infection.

The following dosages are generally used for adults and children over 12 years of age or weighing more than 40 kg:

Mild to moderate infections (e.g., chronic bronchitis): 500 mg amoxicillin with 125 mg clavulanic acid every 8 hours or 875 mg amoxicillin with 125 mg clavulanic acid every 12 hours is usually prescribed.

Severe infections or respiratory tract infections: 875 mg amoxicillin with 125 mg clavulanic acid every 12 hours or 500 mg amoxicillin with 125 mg clavulanic acid every 8 hours. For infections requiring higher doses, such as acute sinusitis or pneumonia, a higher dose may be prescribed.

Co-Amoxiclav dose child per kg: For children under 12 years of age or weighing less than 40 kg, the dosage is usually calculated using a weight-based formula:

The usual dose for mild to moderate infections: 20-40 mg of amoxicillin per kg of the child's weight per day, divided into 2–3 doses. The dose of clavulanic acid is 5 mg per kg of weight.

Severe infections or respiratory tract infections: up to 45 mg of amoxicillin per kg of weight per day, divided into 2 doses.

For very young children and infants, Amoxiclav is often provided as a suspension for easier dosing and swallowing.

When using the antibiotic Amoxiclav, it is important to fully complete the prescribed course of treatment, even if symptoms improve earlier. Premature discontinuation of the course may lead to a recurrence of the infection and the development of antibiotic resistance in bacteria. It is also important to take the antibiotic strictly at the prescribed time to maintain a constant concentration of the drug in the blood.

Contraindications to the use of Amoxiclav

Allergy to penicillins or other beta-lactam antibiotics: Patients with a known allergy to penicillins or other beta-lactam antibiotics such as cephalosporins should not take Amoxiclav as it may cause allergic reactions, possibly even anaphylaxis.

Severe liver dysfunction: A history of severe liver dysfunction, especially associated with previous amoxicillin/clavulanate use, may be a reason to exclude Amoxiclav because of the risk of recurrence of liver problems.

Infectious mononucleosis and lympholeukemia: Use of amoxicillin, a component of Amoxiclav, may result in increased erythematous rash formation in patients with infectious mononucleosis or lympholeukemia; therefore, these conditions are contraindications.

Lyell's and Stevens-Johnson phenomenon: Rare but serious skin reactions with a history of other drugs may increase the risk of such reactions to Amoxiclav.

Amoxiclav: side effects

A variety of side effects may occur while taking Amoxiclav, some of which may require medical attention.

Gastrointestinal disorders: The most common side effects are nausea, vomiting, diarrhea, and abdominal pain. These symptoms may be reduced when the drug is taken with meals.

Allergic reactions: Rash, hives, itching, Quince's edema, and even anaphylactic shock may develop in some patients. Medical attention should be sought immediately at the first signs of an allergy.

Changes in liver function: In rare cases, there may be an increase in liver enzymes, which is usually reversible after discontinuation of the drug. More serious hepatic disorders are rare.

Hematologic changes: Occasionally, the drug may cause changes in the blood such as neutropenia, thrombocytopenia, and anemia. These changes are usually reversible after discontinuation of the drug.

Candidiasis: Prolonged use of Amoxiclav may lead to overgrowth of insensitive organisms, including yeast fungi, which can cause candidiasis such as oral thrush or vaginal thrush.

Neurologic effects: Patients may experience side effects such as dizziness, headache, and rarely, seizures, especially at high doses and in individuals with impaired renal function.

Hyperactivity and insomnia: Some patients may experience increased nervous excitability, insomnia or hyperactivity.

Interaction of Amoxiclav with other medicines

Amoxiclav 625mg, a combination of 500mg Amoxicillin and 125mg Clavulanic acid, is a popular antibiotic for treating a variety of bacterial infections. However, like many other medications, Amoxiclav can interact with other drugs, which requires extreme caution when prescribing it in combination with other medications.

Methotrexate: Amoxiclav may increase the toxicity of methotrexate, a drug used to treat cancer and some autoimmune diseases. This is by decreasing the excretion of methotrexate by the kidneys, which may increase the risk of its side effects.

Anticoagulants: Drugs that affect blood clotting, such as warfarin, may show enhanced effects when co-administered with Amoxiclav. Amoxicillin may alter the intestinal flora, which decreases vitamin K and increases the international normalized ratio (INR) index, which increases the risk of bleeding. In patients taking anticoagulants, close monitoring of blood coagulation indices is recommended during treatment with Amoxiclav.

Allopurinol: Co-administration of Amoxiclav and Allopurinol may increase the risk of allergic skin reactions. Although this effect is not fully understood, patients taking both drugs should be closely monitored for possible dermatologic reactions.

Contraceptives: Amoxiclav, like other antibiotics, can potentially decrease the effectiveness of oral contraceptives. This is due to changes in the gut microflora that may affect estrogen recycling. Women taking oral contraceptives may require additional protection during antibiotic treatment.

Probenecid: Probenecid, used to treat gout, may interact with Amoxiclav, slowing the excretion of amoxicillin from the body and thereby increasing its concentration in the blood. This may require adjusting the dosage of the antibiotic.

Other antibiotics: Combination of Amoxiclav with bacteriostatic antibiotics, such as tetracyclines or macrolides, may be less effective, because the mechanisms of action of these drugs may conflict.

Amoxiclav and alcohol: Combination of Amoxiclav and alcohol is not recommended, as alcohol may increase the side effects of the antibiotic, especially on the gastrointestinal tract, and reduce its effectiveness.

Amoxiclav antibiotic, is a powerful tool in the fight against bacterial infections, but its use should be strictly coordinated with a doctor to avoid the risk of developing bacterial resistance and minimize possible side effects.
